AAA WORKS WHERE THE WEB CAN'T REACH - 60% OF THE PLANET IS NOT CONNECTED - ABOUT 4.6 BILLION PEOPLE

In the U.K. 17m people have poor or no connectivity at home - 525 areas have NO connectivity.

According to Eurostat in December 2016 15% of E.U. households have no connectivity. That's about 112m people.

 

 VITALS, NEWS2 AND PEWS ALARMS with group alerts (FULL Paediatric Early Warning Score (PEWS) with Alarms)

AAA calculates and graphs National Early Warning Score (NEWS V2 Dec 2017) assessments and Paediatric Early Warning Scores (PEWS) from recorded vitals. For PEWS,we use all 5 scoring charts and automatically select the correct one based on the patients age. Medics can now easily see trends. As always, with AAA this is automatically calculated even when 'off-grid'. Alarms on any Vital sign or NEWS score can also be set and members of the group who you wish to be alerted. Alarms are tripped even if the device is off-grid. Full user documentation for both NEWS and PEWS are also available for reading when off-line. If your hospital has it's own risk grading scale, let us know and we will add it just for your hospital.

 

In V1.2 we have added support for Bluetooth LE devices.

 

AAA USES A FORM OF BLOCKCHAIN TECHNOLOGY

Blockchain is a technology that is used in many industries and in many different ways.

There are many forms of databases, programming languages and networks. All work differently and all have their own characteristics. There are also many forms of blockchain that use parts of the technology that suits their needs. For example, if you lose your blockchain key for your health records, would it really be OK to lose you entire health records for ever ?

We use and harness blockchain technology and we understand it. Many don't and hence the hype. If you really want to understand blockchain, ask a developer, not a theorist.

We don't use 'Merkel trees' because we don't have to. We use UTC and time is used as a 'latch'. If a clients transmits at 13:00 and then subsequently sends through transactions created as 10:00, the system rejects the changes. It knows that something is not right. In Medical health records 'double spending' is a necessity not something that should be impossible. Health records have to represent the real world, not a theoretical one. Not that it should but, the same thing can happen to a patient twice. There is often a delay from when actions happen to when they are input into the system. Theoretically, nothing has happened until it is input into the system. The real-world is different.

Download AAA, create a group and move a patient to another group - Watch it re-create the complete chain - immutably. Others say, we do.

Every Group and Action or Task is it's own chain and every chain has an owner. This can be either the patient or the group owner. Each starts with a 'Genesis' block and these chains are combined and cannot be altered. They can only be added to. Tasks and actions are woven into the patient record in their entirety. The chain is complete and contains all the data it needs to exist on it's own. In block chain terms this means AAA actions that are applied are immutable. Just like a financial ledger. As a result, patients (and their data) can be transferred between groups. We do not run a crypto server such as Bitcoin or Ethereum and there is no 'mining' or trading but each patient 'chain' is transferred and copied between groups. It is a chain - to which blocks or treatments are appended.

 

AAA IS AS SECURE AS WE CAN MAKE IT

On Windows Phone and iOS, data is secure to a FIPS140-2 standard. This security standard is approved by both the UK and USA governments for storing personal health records. Your data is secure and legally compliant (WhatsApp is not) - click to see the BBC report. AAA is secure even when off-grid and does not need a connection to authenticate authorised users in the field. We use two factor authentication when you invite group members. Group invitations automatically expire if not accepted within 12Hrs. Every packet of data is 256bit AES encrypted end to end (including chat) and then only ever sent over an HTTPS connection.

  

 

COMMUNICATE AND CONSULT WITH ANYONE ANYWHERE ON THE PLANET SECURELY

You can invite anyone you choose to your group (with two factor authentication) who can then view all details (images, lab results, meds and vitals) for a patient and then issue tasks or jobs to any other medic in the group. Patients and their data can also be transferred to other AAA groups. Role based security is also available. Medics are removed from a group when you choose and all AAA data on their device is purged even if they never connect to the internet again. We can also provide geo-fencing security if required.

We have built in the largest 114 NHS UK hospitals, every ward, contact, telephone number, pager and bleep numbers, department numbers, floorplan with GPS coordinates are ready to use. You can also create a Domiciliary system or your own custom environment. AAA will not only tell you distance and bearing to your patient but also how to get there. If you pin a Live Tile, you can see how busy your group is without even opening the app.

Our initial goal was to solve this problem for UK NHS medics - Turns out that a lot of other people need it as well.

 

What:-

AAA is an “Offline First”, electronic health record and dynamic clinical workflow app that works with its own medical grade, un-tethered wearable vital signs sensor package. AAA helps medics work in hospital, domiciliary environments and care homes even when off-grid. AAA works as expected when it has a connection and, yet users continue to work in their group as normal when unconnected. AAA uses blockchain technology and allows medical institutions and patients to own and share data when needed by creating an immutable healthcare record. AAA is HIPAA and GDPR compliant and can be hosted with the institution or in the cloud. Every AAA data packet is 256bit AES encrypted and then only ever sent over an HTTPS connection.

 

How:- 

AAA combines vital signs data from its wearable sensors package and continuously computes clinical risk every two minutes based on BMA, medically approved and clinically tested scoring methodologies for both adults and children (NEWS and PEWS). The sensor chip we use is medical grade, highly accurate and very advanced. The I.C. was released in Nov 2017 by a USA company and is best in class. The sensor package can be used on its own without the AAA app and does not require a web or cellular connection. The system can be used both in hospitals and for post-discharge patients. An example use case could be an elderly, post discharge patient who has no web connectivity at home and does not use or like modern technology. Using the patient’s vitals data, the device can advise the patient or their caregiver that additional assistance is required or that help should be sought if an abnormal reading is detected.

 

Why:-

Continuous vital sign monitoring significantly reduces patient risks and healthcare costs. The level of patient monitoring outside of the intensive care unit (ICU) is highly variable and often dependent on staffing levels and how busy the hospital is. With AAA, patients are monitored on a 1 to 1 basis, 24x7. The patient and family members will also be reassured that they are being continually monitored. AAA allows medics to set early warning vital sign prediction alerts on patient’s clinical risk or any vital sign threshold and automatically alert selected group members when needed.

 

The sensor:-

Our sensors package is very cheap, waterproof, simple to use and has no keyboard. It has a 5-day re-chargeable battery life. It can also be sterilized so it is re-usable and carries no risk of cross-contamination. It records Heart Rate, Respiration, SP02, movement, fall detection and body temperature. When operated independently of the App, it can also accept pre-programmed alarm limits on any of the measured vital signs. These limits would be set by a medical professional with specific limits for each patient. The system is also able to detect 'false positive' alarms. Our sensor package is ARM Mbed based and open source. The sensor does not legally need FDA approval but in time we will seek approval.

 

A.I. & Machine learning:- 

We are currently running Machine learning experiments on the vitals data we collect with the aim of offering medics suggestions on how to progress the patient's treatment. We developed our first A.I system in 1995 and know that machine learning improves with the size of the data set. Current systems that collect such data are not compatible, are inconsistent and the sensors are very expensive. The sample size they use are also very small. The UK's NHS is still largely paper based as is the majority of the world.

Our goal is to provide M.L based suggestions to medics. For example :- "In 224 patients who had vital signs that match this patient and the presenting complaint was similar, the following actions were taken which delivered improvements and reduced clinical risk ...". We do not wish to replace the medic but to supply them with intelligent tools that help. In areas where no professional advice is available, this feature will be invaluable.

Training machine learning data models is exceptionally 'compute intensive' requiring large cloud based systems to 'number crunch'. Due to advances in Machine learning software however the final models can now be condensed and incorporated into the client software on the smartphone. The model and all its intelligence is then available even when off-grid. This when combined with the rest of the off-line patient record is a very powerful and a unique tool for use anywhere in the world. No web based system can ever do this. AAA is unique. 

Future plans include personalized lifestyle improvement plans based on the user’s actual data.

 

Workflow:-

As the workflow system is dynamic, it does what users need to do, not what we think they need. AAA can also record 'soft signs' such as cognitive function or well-being if required and the questions designed by the group themselves. This is useful in hospitals and also for post discharge patients. Detecting signs of mental health issues such as depression is important.
